Simon Forman (1552-1611) was a noted occultists, herbalist and astrologer under the reigns of Elizabeth I and James I. He also practiced as a physician, basing his diagnoses and treatments on astrology, keeping detailed records of his patient's cases and results. As he had no medical diploma, the barber-surgeon guild prosecuted him and he was imprisoned after the death of one of his patients. He later gained a degree an wentback into practice in 1603.
